Hospital To Home

To be eligible to receive services funded by Hospital to Home, the patient must meet the following criteria:

  • Have a short-term need, which cannot be met through regular community service provision
  • Be assessed as requiring additional services to assist with recuperation or transition to continuing care following an acute episode/presentation
  • Consent to receive additional post acute services under the H2H Program
  • Agree to the forwarding of individual identified data to the Department of Human Services for the purposes of program monitoring and evaluation
  • Assessed by Hospital to Home Program as needing a Hospital to Home funded package to support them in the transition from hospital to home and to assist them in their recuperation
  • Must have received recent acute health care in a hospital
  • Time Frame for referral at discharge:
    • Day only patients up to one week post discharge
    • Inpatient at times of referral
    • Up to 4 weeks post discharge
  • The patient must live in the following municipalities:
    • Northern Grampians Shire Council
    • Ararat Rural City
    • Pyrenees Shire Council
  • The patient must not live in an environment, which would pose a threat to the safety of the service providers visiting the home.
