Community Care Options Team
This web page provides brief information about a range of aged care and disability services provided throughout the Central Grampians sub-region. These programs are auspiced by Grampians Community Health and are based within the Stawell and Ararat Centres.
These services are provided by Central Grampians Community Care Options and the Carer Respite Centre. They provide support to people in the community who are frail, aged or have a disability and to their carers. A carer can be a family member, close friend or neighbour.

Each of the programs has particular eligibility criteria.
These services are a client/carer focused model which provides people with choices about their immediate and long term care and support.
Case Managers and Carer Support Workers travel throughout the Central Grampians Region (Northern Grampians Shire Council, Ararat Rural City, Pyrenees Shire Council) and are available to meet with Service Providers and clients as required.
