   Referrals  
 

Membership at Balgartnie is free, voluntary and is open to people who meet the specific criteria for the program they have been referred to. i.e.; Home Based Outreach Support, Seniors, Day program, Carer Support, etc.
Referrals to Balgartnie are usually assessed and received through Grampians Psychiatric Services, however, the source may initially come from general practitioners or other health professionals, generic services, family, Carers or people may contact the service themselves.

Individuals referred to the Balgartnie Centre undergo a very specific referral process.

Balgartnie will expect that a person with a Serious Mental Illness (SMI) referred to the Centre to undergo a 1:1 orientation and assessment of motivation and needs.

  • Referrals will receive 1:1 contact, several times per week with the same Key Worker for an initial period to arrive at a comprehensive assessment of needs.
  • Assessment of the persons motivation to participate in programs provided.
  • Referral to appropriate program (Adult, Day Program, Dual Diagnosis issues, Community, GCH options, Carer support, etc)

Advantages/Aims:

  1. The referral program should alleviate inappropriate placement within programs and activities.
  2. Potentially reduce complacency and dependence on an individual service or support structure.
  3. Establish positive interests and create a ‘road map' for mainstream community participation.
  4. •  Individualise service provision.
  5. Our referral program will provide choice and control over the direction of the individuals own recovery.

Evaluation:
After, or within, the assessment period, the Individual, Key Worker & Case Manager will reconvene and discuss future service provision or community based options.
